Enrollment Information

Enrollment Packets are available in the Main Office. Registration begins May through June prior to the start of the school year. To find out whether you live within our school's attendance boundary, please enter your home address in the Resident School Identifier (link on left)

Age Requirements
For Kindergarten, the child must be 5-years old on or before September 1. Transitional Kindergarten (TK) is designed for students who turn 5-years old from September 1 to  December 2.

Registration Requirements
Return a completed Enrollment Packet to the Main Office. Please bring your child with youOnly completed packets will be accepted.  The following must be brought with you in order to register:

  • Student Enrollment Form (3 pages)
  • Emergency Card
  • Ethnicity Identification
  • Migrant Education Program - Family Questionnaire
  • Student Residency Questionnaire
  • American Indian-Alaskan Native Letter (Title VII)
  • Oral Health Assessment/Waiver Request Form
  • Permanent Health History (white card)
  • Physical (Orange Card)
  • Individualized Education Program [IEP] (If applicable)
  • Court Orders(s) (If applicable)

You will also need to bring the following items:

  • Copy of parent ID
  • Proof of Residence Utility bill (excluding telephone bill) or Mortgage document or signed lease agreement
  • Proof of Age Birth Certificate (the recorded copy, not the souvenir from the hospital) or Baptismal records or Passport
  • Immunization Record – Immunization must be current. California State Law immunizations are required.
  • 4 Polio (3 Polio meets there requirement if the 3rd Polio was given on or after the 4th birthday)
  • 5 DTP (4 DTP meets the requirement if the 4th DTP was given on or after the 4th birthday)
  • 2 MMR (1 measles only + 1 MMR meets the requirement if both were given on or after the 1st birthday)
  • 3 Hepatitis B (3 doses at any age)
  • 1 Varicella (history of Chicken Pox Disease, documented on record by a physician, meets the Varicella requirement)
  • 1 Maxtoux TB Test

 


GETTING READY FOR THE FIRST DAY OF SCHOOL


Room Assignments
Room assignments will be posted at the front entrance on Silvius Ave. at 6:00 pm the evening before the first day of school.

Uniforms
There are no uniforms, however proper school attire is required: no sandals, no skate shoes, girls wear shorts under skirts/dresses.

School Supplies
Basic supplies will be provided by the school such as paper, pencils, crayons, etc. If the teachers request any specific supplies they will send a letter home with your child. 

Special Needs
Make the main office aware of your child’s special needs, for example Individualized Education Plan (IEP), medications, allergies, etc.
